IDENTIFICATION REQUIREMENTS
Applicants must provide Access Copyright with photocopies or scans of sample(s) of their work as they appear in the publication. Work samples can be any items you have published to which you retain copyright or reprographic rights. The rights you retain to your works will be specified in the agreement between you and your publisher. We do not require a complete works list during registration, only what is indicated below:
** If you are applying as both a writer and a visual artist, you cannot submit the
same sample(s).
Only ONE sample is required if:
You have published a book with an ISBN and the book is not self-published.
Please mail or email ONE work sample (per rights holder type, i.e. writer
and visual artist) to Access Copyright that includes a photocopy or scan of each
of the following:
- The book cover
- The copyright page of the book with your name clearly credited as the writer/visual
artist
- For Visual Artists - one page of your visual work as it appears in the book
At least THREE samples are required if:
Your work is an image, poem, article, short story, chapter, or any other type of
excerpt, published in newspapers, magazines, journals, books with ISBNs, books without
ISBNs or self-published books. Please mail or email THREE
work samples (per rights holder type, i.e. writer and visual artist) to Access Copyright.
Work samples cannot appear in the same publication unless they have been published
in three separate issues. One of your work samples must be from a publication with
an ISBN or an ISSN in order to be eligible to register with Access Copyright as
a creator affiliate. If you are submitting work samples published in an online-only newspaper, magazine or journal, the publication must have a valid ISSN.
For each sample, please provide photocopies or scans of the following from the published
edition of the work:
- The cover of the work (e.g., cover of the magazine, newspaper, journal or book that
your work appears in
- The copyright page of the book or masthead of the serial publication
- For Writers – the first page your work appears on in the publication
- For Visual Artists - one page of your visual work as it appears in the publication
with a credit to you as the visual artist.
Once you have completed this application by clicking on the ‘Submit’ button below,
you will receive an email with instructions on how to submit your photocopies or
scans of your work sample(s). Please submit your sample(s) with your identification
and Social Insurance Number.
By affiliating with Access Copyright, you appoint Access Copyright as your collective society to license and administer on a collective basis the copyright in your Works in accordance with the terms and conditions and this form, as completed by you (together, the "Agreement"). Please read the terms and conditions carefully, as they form part of your Agreement with Access Copyright.
By completing this form and clicking the "Submit" button below, you provide your electronic signature signifying your acceptance of the Agreement. You warrant that all of the information provided in the form is accurate and true. You agree to compensate Access Copyright in full for any loss resulting from any proven claims made against Access Copyright caused, in whole or in part, by any misrepresentation by you as to the extent to which you own or control the copyright in the Works covered by this Agreement and your failure to advise Access Copyright that you are no longer legally entitled to receive royalties for the Works covered by this Agreement. Access Copyright will notify you by email after your application is approved regarding how to obtain your signed pdf version of the Creator Affiliation Agreement to keep for your records.
